Rank List & Withdrawal

Below are instructions on how to submit your rank list, revise your existing
list and withdraw from the Match.
Submitting
Your Rank List For the First Time ...
All applicants should use this form to submit their rank list. All lists
must be received in our office by 12pm (PST) the day of the match deadline.
There is no fee for submitting your rank list for the first time. You will
receive a confirmation email with your rank list entries within one business
day.

Withdrawing
From the Match ...
Some applicants may decide that they are no longer interested in the
specialty for which they registered. If you change your mind, get an earlier
appointment, or do not want to submit a rank list for any other reason,
send us your WITHDRAWAL
FORM.

Changes
To Your Rank List ...
Changes to your rank list after certification will be accepted up until
the Match deadline. If you would like to make changes, please fill out
the REVISED
RANK LIST FORM. Please note that you must either fax
or mail the revised list to our office. A $100 change fee is applicable
for each time you change your list. If you are faxing your revised list,
please use a VISA or MasterCard.

NOTE: There is no strategy for arranging your rank list other than by
order of your preference. Arranging your rank list in any other way can
only hurt your chances of matching to your most desired choice.
All rank lists are DUE by 12:00PM (noon, PST) on the day of the deadline.
Late rank lists will NOT be accepted.
