Registration for Ophthalmology Fellowship Match is open from June until November. Late applicants should take into consideration program deadlines. Please scroll down to download a registration form.

Registration Confirmation

Once registered, you will receive confirmation e-mail with your 5-digit match registration number, login to access the full-version of the program directory, and link for secure online payment. Additional match information can be found on this website.

You may pay the $250 non-refundable registration fee by:

• Personal check

Submitting your form

Completed registration forms should be submitted electronically by clicking the “Submit Online” Button at the bottom of page one of the form.
