About CAS

The Central Application Service (CAS) distributes applications to residency and fellowship training programs online. Applicants are required to fill out the online CAS application form, provide a set of required documents and select programs for distribution. A separate fee is required for CAS distributions.

CAS will distribute complete applications to programs. For Ophthalmic Plastic & Reconstructive Surgery Fellowship an application is complete when the following criteria are met:

Complete CAS Application form (on-line submission);
CAS Distribution List and Payment (on-line submission);
Medical School Transcripts;
USMLE Scores
Three (3) letters of reference (one from the department Chair)
CV (please see format in CAS instructions manual)
