Applying To Programs

Step 1: Gather the following required documents:

CV
Personal Statement
Maximum of three letters of reference

Step 2: Mail required materials to

Not avaialble for weekend or holiday delivery.

Step 3: Select your programs

Login to your SF Match account and choose your programs from your "Add My Programs" list then click on "Apply To Programs" to submit your distribution choices. Your contact information will be made available to programs you have requested your application to be sent to.

There are additional fees for participating in the CAS service. Distribution fees increase progressively according to the number of applications you want sent.

Payments for distribution fees may be paid by Visa or MasterCard or check. Credit card payments take up to two-business days to be processed. Check payments may take up to 7-business days. Check payments must be made to Rhinology Fellowship Match and mailed with check coupon to:
