Original CAS Documents - return request
Our office will return original CAS documents (via regular mail) at the
applicant's request. There is a $20 fee for this service. Please complete,
print, sign,
and return this form to have your documents returned to you. Documents will
NOT be returned until AFTER the match takes place. All requests
must be in writing; we cannot help you over the phone. Please note that our
office will store your originals documents for one year after the Match is
concluded.
