About CAS

The Central Application Service (CAS) distributes applications to residency and fellowship training programs online. Applicants are required fill out a CAS application form, provide a set of required documents, and select programs for distribution. A separate fee is required for CAS distributions.

Applicants are responsible for assembling a complete CAS application package which includes the following documents:

  • CAS Application Form (online form available after registration is concluded)
  • College Transcript(s)*
  • Medical School Transcript(s)
  • USMLE Score Report or Transcript(s)
  • ECFMG Certificate (applicable to International Medical Graduates)**
  • Three letters of reference
  • Dean's letter***
Some programs may require additional support documents. Documents that are not listed in the CAS instructions manual must be sent directly to the program..

*International Medical Graduates who did not attend college prior to medical school must submit a letter of explanation for missing college transcript.
**Applicable to International Medical Graduates only. Not required for the initial distribution to programs; however, a letter stating the status of ECFMG certification and expected date of completion must be included with the initial application.

The Dean's Letter is not a required document for initial distribution to programs as they will be released by Medical Schools on November 1st. International Medical Graduates who do not have a Dean's letter, may submit a 4th letter of reference instead.
